-Farmakorezistentní- hypertenze

Popsán je případ 49letého hypertonika s tzv. farmakorezistentní hypertenzí, odeslaného z odborné interní ambulance se čtyřkombinací atenolol, amlodipin, cilazapril a moxonidin s TK 190/115 mmHg. Po úpravě terapie na kombinaci betaxololu, amlodipinu a trandolaprilu s diuretikem hydrochlorothiazidem došlo k dlouhodobé normalizaci TK na hodnoty 120-138/82-88 mmHg. Po roce došlo opět k výraznému zvýšení TK, po kontrolovaném podání léků na ambulanci byla potvrzena jejich dobrá účinnost. Důvodem non-compliance pacienta byla ztráta zaměstnání a pobírání nemocenské, tedy sociální non-compliance.

Je popsán případ devětačtyřicetiletého hypertonika s tzv. farmakorezistentní hypertenzí, odeslaného z odborné interní ambulance se čtyřkombinací atenolol, amlodipin, cilazapril a moxonidin s TK 190/115 mmHg. -Pharmacoresistant- hypertension

The case of -pharmacoresistant- hypertension in 49-year old patient is described. The patient was sent from internal medicine specialist with 4-combination of atenolol, amlodipine, cilazapril and moxonidine and blood pressure (BP) was 190/115 mmHg. The therapy was changed and hydrochlorothiazide was added to betaxolol, amlodipine and trandolapril. The patient was fully compensated for one year with blood pressure values 120-138/82-88 mmHg. Then decompensation occurred again, but in connection with controlled administration in the outpatient ward the BP returned to normal. The non-compliance resulted from the loss of occupation - that is a social non-compliance.
