Při interakci léčiv s cílovými strukturami (receptory) v organizmu se uplatňují účinněji ty látky, jejichž struktura odpovídá aktivnímu místu farmakologického receptoru. Vyskytuje-li se léčivo ve dvou (nebo více) prostorových izomerech (enantiomerech), pak se lépe váže na receptor ten izomer, jehož struktura koresponduje dokonaleji struktuře aktivního místa. Druhý stereoizomer může být farmakologicky neúčinný, méně účinný nebo může vykazovat zcela jiný (i nežádoucí) účinek. Tyto skutečnosti jsou dokumentovány na příkladech dexketoprofenu, citalopramu, omeprazolu a dextrometorfanu.

Drugs and chirality

Selectivity of the drug-receptor interaction is given by the complementarity of the spatial structures of the drug and of the receptor active site. In the case the drug is present in two (or more) stereoisomers, then, an isomer whose structure fits better to the active site of the receptor is more efficient. The other stereoisomer (enantiomer) may be entirely inactive, may be of lower efficacy or may even possess different pharmacological properties. These possibilities are documented here in examples as dexketoprofene, citalopram, omeprazole and dextromethorphan.
