Komplikované stadium Parkinsonovy nemoci je u řady pacientů patrné již po 7-8 letech léčby, a znamená rozvoj fluktuací stavu charakteru on-off, dyskinezí a občasného freezingu. Možností, jak terapeuticky zasáhnout je více. Patrně nejšířeji užívanou možností léčebného ovlivnění motorických komplikací Parkinsonovy nemoci je přidání agonistů dopaminu a COMT inhibitorů do léčby. Existuje několik -scénářů- použití těchto léků, a každý je nejlépe použitelný ve specifické situaci.

TREATMENT OF COMPLICATED PARKINSON-S DISEASE

Advanced, complicated stage of Parkinson´s disease can occur in many patients after as early 7 to 8 years of systematic treatment. It is characterised by the onset and progression of motor fluctuations of the -on-off- type, dyskinesias and freezing. There are several possibilities of therapeutic intervention. Probably the one most frequently used is pharmacologic manipulation using dopaminergic agonists and COMT inhibitors. There are several -scenarios- of how to introduce and use these drugs, and each of them is best for a given situation.
