The aim of rational pharmacotherapy of epilepsy is to maximize effectiveness as well as to minimize side effects. In comparison with old antiepileptic drugs (1st and 2nd generation), the side effects and interactions of new „3rd generation“ antiepileptics are less known. The overview of efficacy and tolerability of new antiepileptic drugs (gabapentin, lamotrigine, topiramate, levetiracetam, occarbazepine, tiagabine, zonisamide) is based on a metanalysis of 1 462 publications. In terms of interactions, the slow degradation of lamotrigine due to a combination with valproate is stressed.
