Multiple sclerosis (MS) is a chronic inflammatory demyelinating disease of the central nervous system (CNS) with the prevalence in the Czech Republic 1:1 000. Almost all lactating women receive some medications immediately postpartum and during breastfeeding. Almost all medications transfer into human milk. The use of medications in breastfeeding mothers always requires a careful assessment of risk versus benefit, on the other hand interrupting breastfeeding for unsound reasoning such as a medication should be avoided if at all possible.

Sepsis is defined as an inflammatory mechanism by an organism in response to a pathogen. The controversies of sepsis in adults in comparison with sepsis in children are discussed in this paper. The fact that the clinical symptoms or laboratory parameters of sepsis do not exist. Definition and etiology of sepsis and SIRS (systemic inflammatory response syndrom) in children are different from that in adults. Sepsis therapy must be complete in consideration to MODS (multiple organ dysfunction syndrom) and other factors. We discussed basic facts in different therapies of sepsis in adults and children.

Systemic antifungal agents are useful to treat systemic mycoses. Drugs include polyens, antimetabolites, azoles, echinocandins, and others. Main representative of polyens is amphotericin B. To reduce its toxicity, lipid-based formulations were developed. Amphotericin B molecule is bound to lipid vehicle, namely liposome (liposomal amphotericin B), cholesterol disks, with its forms of lipid complex (ABLC), and amphotericin B colloidal dispersion with cholesterol (ABCD). Risk perceptions in the antiaggregation preventive therapy

There is high level of evidence available to prove the efficacy of preventive antithrombotic therapy in the reduction of acute vascular events. On the other hand, there is considerable risk associated with this therapy. Bleeding and other adverse drug reactions often limit the usage of the basic antiplatelet drug, that is aspirin. Clopidogrel has proved better tolerance as well as effectivity in this setting. The inadequate drug risk perception may negatively influence the prescriptive decisions of doctors and patients´ adherence to the treatment. The authors of the review article aimed to show the limits of the chosen drugs.

The evaluation of compliance in patients on aspirin prevention

Preventive antithrombotic therapy plays an important role in the prevention of acute complications of cardiovascular (CV) diseases. The low dose aspirin (acetylsalicylic acid) as an antiplatelet agent is still a key drug in primary and secondary CV prevention. Its efficacy in the reduction of clinical events was proved by EBM (evidence based medicine). However, it has been shown that aspirin doesn`t always provide the expected protection. We often face the problem of the failure of aspirin prevention. A clinically and biochemically defined term called aspirin resistance has appeared. Its real prevalence and clinical importance still remain uncertain....

ALFIMEPRASE - A NOVEL, DIRECT-ACTING FIBRINOLYTIC AGENT

This article describes the continuing investigation of alfimeprase, a novel direct-acting thrombolytic agent for its indications - the treatment of acute peripheral arterial occlusion, stroke, and catheter occlusion. Alfimeprase dissolves blood clots faster than current therapies, degrades fibrin directly.
