Aims: To evaluate the possibility of monitoring the effect of anti TNFα therapy in patients with active rheumatoid arthritis by multiplex assessment of serum levels of different biomarkers which reflects underlying pathogenic mechanisms and activity of this disease. Methods: A group of 23 patients with active rheumatoid arthritis, 20 female, 3 male, average age 48.6 years ± 10.3, average weight 68,5 ± 13 kg was evaluated prior therapy and after 14–16 weeks of therapy with TNFα blocking agent adalimumab 40 mg subcutaneously every two weeks (No = 21) and etanercept 25 mg subcutaneously twice weekly (No = 2). Of 23 patients,...

Knowing the function of B cell receptor and its signaling cascade enabled the development of the new molecules that inhibit the enzymes of this patway. Ibrutinib is the inhibitor of Bruton´s tyrosin kinase which has the key role in the transmission of signals from this receptor. Nowadays, ibrutinib is approved for the treatment of relapsed or refractory B-chronic lymphocytic leukemia, for the first-line therapy of chronic lymphocytic leukemia with the deletion of the 17p deletion or mutation in the TP53 gen. Moreover, it is approved for the threatment of follicular lymphoma and Waldenström’s macroglobulinemia. In our work, there is a closer describtion...

Venous thromboembolism (VTE) is due to its incidence, morbidity and mortality an important global socioeconomical problem. Antithrombotic prevention and treatment are one of the most common areas in moderne medicine. TEN is still the frequent causes of mortality and morbidity. We are currently used in this field heparins, pentasaccharide and warfarin. Warfarin despites undoubted effect, especially for its interindividual variability, the risk of drug and food interactions and a narrow therapeutic window. The development of new, effective oral anticoagulants (NOAC) focused mainly on direct thrombin inhibitors and direct factor Xa inhibitor. From the...

The theoretical part of this article summarizes the antimicrobial potential of silver, including well-known historical facts, and its application in the form of nanoparticles within colloidal systems or as a part of nanocomposites. The outcomes of biological activity of colloidal silver tested at the Faculty of Medicine and Dentistry and Faculty of Science, Palacky University Olomouc, are also summarized in this article. The tests undertaken were focused on silver nanoparticles and their antibacterial and antifungal activity, cytotoxicity and synergic effect with selected antibiotics, and the development and testing of nanocomposites with possible...

Neuropathic pain is among difficult-to-treat pain conditions. Its occurrence in the population increases with increasing age. Anticonvulsants and antidepressants are the fundamental drugs in treating this pain. In case there is no adequate relief of pain, second-choice drugs are the next option, including, in particular, opioids, with some of them having a higher affinity to neuropathic pain. They particularly include oxycodone and tapentadol, with some papers also reporting buprenorphine. Treatment is always initiated with the lowest doses that are titrated to an effective dose. Currently, however, the ceiling effect of opioids is being applied that...

Expanding of resistance of still susceptible strains of bacteria to antibiotics, associated with the increasing consumption of antibiotics in both human and veterinary medicine, is occurring faster than the marketing approvals and the implementation of new antibiotic molecules to clinical practice. The development of new antibiotic molecules is troublesome, expensive and timeconsuming. Inspite of this, 30 new antibiotic molecules were registered in the last 15 years and more than 10 molecules are in the process of phase III clinical trials. Therefore we can expect further new marketing approvals in the field of antimicrobial drugs in the upcoming years....

The paper defines the basic approach to patients with sepsis or potentially developing sepsis from the perspective of clinical microbiology. Also characterized are the principles of collecting biological samples, their transport and potential storage. At the same time, the basic algorithms for initial antibiotic therapy are presented.

The case report showcases a complication of amiodarone treatment – type 2 amiodarone induced thyrotoxicosis (AIT) with a fatal outcome. A 79-year old patient was being treated with the culprit drug for paroxysmal atrial fibrillation. The treatment was complicated by an atypical clinical presentation of AIT with altered mental status, qualitative change of consciousness and heart failure resulting in cardiac arrest. A successful CPR was performed and the patient was put on mechanical ventilation. An aggressive course of thyrostatic treatment along with corticosteroids was started early on. Due to little to no clinical or laboratory response an...
