Aim of study: The prevalence of hypertension is 1,5 to 2-fold higher in patients with diabetes. Incidence of hypertension in association with diabetes increases risk of development of serious cardiovascular complication. The aim of our study was to evaluate clinical condition and pharmacotherapy of patients with essential hypertension (EH) and diabetes mellitus (DM) in comparison to patients without diabetes. In both groups, we also compared control of blood pressure (BP), which was measured by conventional technique and 24-hour blood pressure monitoring (ABPM). Method: Medical records of 95 patients with treated hypertension (40 patients with DM...

Aim: Bone metastases occur in up to 70 % of patients with breast cancer and represent high risk of SRE development (Skeletal Related Events) and bone pain which result in decreasing quality of life. Bisphosphonates are potent compounds which decline the occurrence and relapse of SRE. The aim of study was to evaluate the therapy with bisphosphonates in patients with metastatic bone disease from breast cancer. Methods: Retrospective observation study is involving two university oncological centers in Czech Republic. The data for analysis were obtained from medical records of 130 patients with bone metastases from breast cancer who died between...

Head and neck tumours are conditions in which the outcome of therapy is determined by the ability to achieve locoregional control of the tumour. The treatment of an advanced head and neck tumour with radiochemotherapy produces a good therapeutic response, however, in 40 percent of patients recurrence occurs. Concurrent administration of cetuximab and radiotherapy in advanced carcinomas of this region significantly prolongs time to progression (from 14.9 months to 24.4 months). A combination of cetuximab and chemotherapy in relapsing disease following primary therapy extends survival by 2.5 months.

A case is reported of serious acquired aortic valve defect in a 51-year-old man who has never experienced rheumatic fever nor any other inflammatory disease of the endocardium. In the course of a long-term follow-up for WPW syndrome, the valvular defect was not present and it must have developed during the past four years when the patient used third generation SSRI antidepressants for two years as well as benzodiazepine anxiolytics (Xanax tablets). An overview of literature on acquired valvular defects following the use of anorectics-antidepressants is presented. Acquired valvular defects due to the action of excess serotonin in tissues seem to result...

The improvement of the health care quality represents a common goal of professionals at different levels of health care and a prominent public priority. The inseparable part of the health care is pharmacotherapy. The unbiased comparison and evaluation of trends of pharmacotherapy quality require a quantitative analysis using quality indicators. Quality indicator is defined as a measurable element of prescribing performance for which there is evidence or consensus that it can be used to assess quality, and hence in changing the quality of care provided. They can be classified according to their focus (structure, process and outcome indicators) and their...

The number of people over age 65 is rapidly increasing, and in less than 50 years, one of every three people in Czech Republic will be older than 65. Blood pressure, particularly systolic blood pressure, tends to increase progressively with age. Elderly people with hypertension have a greater risk of cardiovascular disease. The benefits of treating hypertension in the elderly have recently been documented. The choice of antihypertensive therapy should be based on the presence of concomitant conditions and should by individualized. First-line therapy are diuretics, ACE inhibitors, angiotensin II receptor blockers and calcium channel blockers. Therapy...

Decubitus is a major concern in geriatric medicine since 70 percent of all decubitus cases occur in persons over 70. It is closely related to immobilization syndrome which, in addition to instability, incontinence, and intellectual impairment, is one of the so-called „major geriatric syndromes“. These mainly occur in late old age (over age 75) as a complement to the syndromological picture of general multimorbidity. They are commonly accompanied by loss of self-sufficiency. The issues of aetiopathogenesis, clinical picture including classification and complications are discussed. Particular emphasis is placed on the possible role of scales...

Probiotics are live microbial cultures that can improve a human health and reduce a risk of human desease. Probiotics are used in many gastrointestinal disorders, mostly inflammatory bowel deseases, but an interest in using them in liver demage increases. Probiotics can improve minimal hepatic encefalopathy and prevent infectious complications in these risk and critically ill patients. In our study we showed that E. coli Nissle can in patients with liver cirrhossis normalize intestinal microflora, decrease a level of endotoxemia and prove liver function.

Hepatoprotectives are substances with potential positive effect on liver cell regeneration or to slowering of pathological processes in chronic liver diseases. These effects were confirmed in experimental studies with many substances; nevertheless the clinical use is limited. Ursodeoxycholic acid possess the positive effect in patients with primary biliary cirrhosis or other cholestatic diseases. This is a treatment of choice in this indication. Similar effect can be attributed to S-adenosyl methionine. Recently many herbal extracts are very popular. In practical use is silymarine, which can slow down the fibrotic process by antioxidant characteristcs. Another...
