Klin Farmakol Farm. 2018;32(4):3-9 | DOI: 10.36290/far.2018.023

Aims: Investigator-initiated (“academic”) clinical trials (IICTs) are designed and carried out without a participation of any commercialsubject. IICTs adopt a patient-oriented approach and attempt to answer relevant questions from the clinical practice thatmay not be attractive for the pharmaceutical companies. The results obtained by IICTs are considered independent and clinicallysignificant. The purpose of this study was to analyse data from the clinical trial public registers to describe a current status of IICTsand their financial support in the Czech Republic (CR).Methods: The search was performed in four international clinical trials...

Current pharmacotherapy of tuberculosis

Jiří Wallenfels, Martina Vaąáková, Ivan Solovič

Klin Farmakol Farm. 2018;32(4):10-14 | DOI: 10.36290/far.2018.032

Tuberculosis still represents global threat, with respect both to the number of infected, ill and dead individuals. Increasing incidenceof resistant forms of tuberculosis with significantly worse treatment outcomes compared with sensitive tuberculosisposes a concern. New antituberculotics and antibiotics prescribed until now mainly for other infections are starting to be usedfor treatment of resistant tuberculosis. New, more effective, shorter and better tolerated treatment coarses are investigated. Thecurrent epidemiologic situation in the Czech Republic is relatively favourable, it is appropriate to discuss about the obligation tohospitalize all...

COPD treatment

Norbert Pauk

Klin Farmakol Farm. 2018;32(4):15-20 | DOI: 10.36290/far.2018.024

Chronic Obstructive Pulmonary Disease (COPD) is a treatable, preventable and clinically heterogeneous syndrome which manifestswith various lung symptoms and is associated with various comorbidities. This article deals mainly with the pharmacologicaltreatment of stable COPD according to the current recommendations of the Global Initiative for Chronic Obstructive Lung Disease(GOLD) and the valid recommendations of the Czech Pneumological and Phthisiological Society. Pharmacotherapy of COPD isprimarily based on long-term inhaled bronchodilators (LABD). It is a group of long-term (or ultra-long-term) inhaled anticholinergics(LAMA or U-LAMA) and inhaled...

Drug-induced lung injury

Klin Farmakol Farm. 2018;32(4):21-31 | DOI: 10.36290/far.2018.025

The treatment of various types of diseases can be associated with a number of adverse side effects of drugs. Drugs that can cause lungdisease are constantly on the increase. This is due to not only the development of new drugs, but also the use of modern diagnosticmeans allowing more accurate investigation. The mainstay of diagnosing drug-induced injury is the identification of a drug that, withcertain probability, could have induced lung injury. To identify it among other drugs that a patient is taking as well as to distinguishdrug-induced injury from a possible exacerbation of an existing lung disease is difficult. The most common drugs that cause...

Antibody‑Drug Conjugates

Miroslav Miletín

Klin Farmakol Farm. 2018;32(4):32-37 | DOI: 10.36290/far.2018.033

Monoclonal Antibodies (MAbs) belong among the fastest growing types of biologics for a variety of different indications andemploying different mechanisms of action. The trend in the development of new MAbs is to modify their basic structure, leadingto an improvement in the therapeutic profile or at least pharmacokinetic properties. In addition to the basic types of antibodies, their biosynthetic and semi-synthetic modifications are currently available for clinical use. Biosynthetic interventions includee.g. changes in the sugar moiety of the molecule, or the changes of several amino acids in constant regions of the MAb. Variableantibody fragments alone,...

Is the calcium supplementation in palliative care always unnecessary?

Judita Kudělová, Michaela ©lesingerová, Jana Gregorová

Klin Farmakol Farm. 2018;32(4):38-41 | DOI: 10.36290/far.2018.026

Disruption of calcium homeostasis is a common complication in oncological patients, particularly in patients with metastaticbone disorder. Imbalance of bone metabolism may lead to hypercalcaemia or to less common hypocalcaemia. The followingarticle presents a case report of a patient with diagnosis of a lung cancer. Significant hypocalcaemia was detected at the time ofhospital admission. After initial intravenous mineral supplementation therapy, the deficiency of vitamin D was considered withinthe differential diagnosis of hypocalcaemia. Subsequent vitamin D metabolite blood test confirmed serious hypovitaminosis.Based on the laboratory findings, mineral...
